However, contrary to what the name implies, you can have PCOS and not have any polycystic ovaries. Other common side effects of PCOS include acne, facial hair (also known as hirsutism), hair loss, insulin resistance, fatigue, and anxiety. What Is IUI?

Intrauterine insemination (IUI) is a fertility treatment aimed at increasing the chances of conception. It is often recommended for couples with unexplained infertility, male factor infertility (such as low sperm count or poor sperm motility), or individuals using donor sperm.

The IUI process is straightforward and involves three main steps. First, ovulation is tracked to identify the optimal fertile period. Next, semen is collected, washed, and prepared in a laboratory to concentrate the healthiest sperm. Finally, using a thin, flexible tube called an insemination catheter, the sperm is directly injected into the uterus.

A low-complexity procedure helps overcome barriers to conception by delivering a concentrated dose of sperm at the peak of ovulation. Typically, IUI involves two treatments: one at 12 hours and another at 36 hours after administering hormone medications, ensuring that sperm is in the best position to fertilize the egg. This approach improves the chances of pregnancy by providing a more direct path for sperm to reach the egg.

 

Success Rates of IUI with PCOS

In a study assessing the pregnancy outcomes of infertile women with polycystic ovary syndrome (PCOS) undergoing IUI after various attempts at ovulation induction (OI), researchers found that the number of previous OI cycles did not significantly affect IUI success rates. The clinical pregnancy rates and live birth rates remained comparable across different groups of patients, regardless of whether they had undergone none, one to two, or three or more previous OI cycles. 

Approximately 98% of pregnancies occurred within the first three cycles of IUI. This suggests that IUI can be an effective, less invasive alternative for PCOS patients before progressing to more invasive treatments like in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI). Given these findings, women with PCOS can consider three IUI attempts before transitioning to a more invasive procedure.

 

Factors Influencing IUI Success Rates with PCOS

Several factors can impact the success rates of IUI for women with PCOS:

  1. Ovulation Regularity: Women with PCOS may have irregular ovulation, which can affect the timing and success of IUI. Successful ovulation induction through medications like Clomiphene Citrate or Letrozole can significantly enhance the chances of a successful IUI.
  2. Age: Age plays a crucial role in fertility. Women under 35 generally have higher success rates with IUI compared to those over 35. 
  3. Weight Management: Maintaining a healthy weight through diet and exercise can significantly improve ovulation and enhance the effectiveness of IUI. For overweight women with PCOS, weight loss has been linked to better fertility and more successful IUI outcomes. However, losing weight can be particularly challenging for those with PCOS, so remember not to get discouraged. Your fertility doctor can help you develop a plan to address these challenges. It’s important to note that individuals with PCOS are 3 to 6 times more likely to experience eating disorders compared to those without the condition, which can further complicate weight management efforts.
  4. Underlying Health Conditions: Addressing other health conditions related to PCOS, such as insulin resistance, can positively impact IUI success rates. Medications like Metformin may be prescribed to manage insulin levels and enhance fertility. There are also more natural ways to help improve insulin resistance, such as myoinositol, and lifestyle changes like diet and exercise. 
  5. Semen Quality: The quality of the sperm used in IUI can affect success rates. Ensuring that sperm is well-prepared and healthy is crucial for maximizing the chances of conception.
